On this page you will find all of the required forms for new clients. Please print and complete these forms and then bring them to your first appointment. Doing so will maximize your time with the therapist.

 

For the new client form please choose the ADULT form for clients 18 and older and the MINOR form for clients under the age of 18. Guardians must sign forms on behalf of all clients under the age of 18.

 

The Therapeutic Agreement form is required for all new clients. We request that if this form is being filled out for a minor client, that both the guardian and the minor initial the form so that all parties understand the information contained in the form. For couples counseling, only one therapeutic agreement form is required with both parties initialing.

 

All new clients must complete the HIPAA Privacy Notice form and the Billing and Payment Agreement form. Guardians should sign these forms on the behalf of minor clients.
